HEALTH CONNECTIONS MENDIP
Health Connectors are a part of your GP practice staff team and you can make an appointment to talk to a Health Connector (your GP reception desk can advise you how to contact us). Health Connections can work with you in a non-medical way to help your health and wellbeing, working in a number of ways to suit you. This could be helping you to meet others, being connected to other services, getting more involved in your community, helpful training, help to get online, building your own confidence or helping you to support others.
We run groups such as Talking Cafes and Healthy Lifestyles Programmes. You might like to do some short training with us as a Community Connector – you can then signpost others to help that is freely available to all in the community. There are so many ways we can help you to become involved in your own wellbeing it all starts with a call or visit to our Health Connections Website for more information. Our Service is free of charge and open to patients of Mendip GP Practices. Do contact us to chat through what support you might need or how you can get involved.
