Welcome to Oakhill Surgery

We are pleased to accept registration requests from patients who are living within the area indicated in the map below:

Please see our practice boundary and check to see if your postcode is within our catchment area.

We accept patients within a set boundary; this is to ensure that we are able to give a good quality of service to our patients.

Registering as a patient

You can register at the practice ONLINE or by completing a registration pack which can be collected from reception at the Surgery.

Change of Personal Details

Please inform reception if you change your name, status, address or telephone number, especially mobile telephone numbers. You will be asked to complete a form to ensure that your records contain up-to-date information or you can complete our Change your Contact Details form.
