If you let us know we can code this on your medical records and you will be invited to any carers events that we organise.

The Practice has a dedicated notice board for Carers located in the reception area.
Health Connections Mendip (HCM)
Based in the Somerset district of Mendip, the initiative offers a range of services, including community development, peer support, a website directory, EMIS patient record system, social prescribing, one-to-one support and Community Connector training to enable people living in Mendip to improve personal and community resilience.

Somerset Carers Service
If you regularly look after someone in a caring capacity, the Somerset Carers Service can support you with advice and guidance. Caring for another person is very rewarding, but can be overwhelming and lonely at times.

This service provides Carers Agents who are Somerset based individuals with a wide knowledge base of local services and information that are invaluable to carers. They are on hand to offer personal guidance and support.
